Form 8821 for Deceased Taxpayers with Appointed Executor

Hello! I am working with the daughter of my deceased clients (husband shot wife then shot self in 2018) and she has been appointed executor of their estates. We have already filed Form 56 for each individual. I had been working with the husband for several years and he was behind in filing. I was never able to get tax documents from him for 2016 and 2017 so I will need to request transcripts. How EXACTLY does one fill out the 8821? Do I use the TAXPAYER name, ,social security number, address, and telephone and have the executor sign? Do I indicate anywhere on the forms that they are DECEASED? I have scoured the instructions several times so any experience or assistance would be greatly appreciated.
